I see an error page after submitting...

Seems so. I wanted to complain to address shown, when the delist request
above responded with an error. I can't reproduce this, all domains I found
and are now correct, seem to get delisted when checking evidence :)

I don't remember the error now, neither do I remember what did the evidence
about the domain I delisted yesterday... I'm just sure it wasn't the "domain
appears to be fixed".

... I just got two errors (after form submission and after mailing the
mentioned address) so I tried to report that...

If I'll be able to reproduce, I'll report...
