I've heard a number of reports of this happening with Mailman, so that

One could set Mailman's option to set the "moderated" flag on all users
when they subscribe, then, upon approving the first post by a user, I
believe the moderation page provides a "turn off the moderated flag"
option (or something with similar effect). Of course, all the spammers
then really have to do is automate subscribing, sniff list traffic for
legit posts, and forge the from address. But that might be enough
effort to not be worth it (yet).
